Root Cause Analysis and Incident Investigation

Preventing the recurrence of accidents and equipment failures is at the heart of root cause analysis and incident investigation. There are common pitfalls in such investigations however, such as halting the review once an immediate cause has been identified. At best, remedying the immediate cause will help prevent only very similar incidents from occurring. It is an understanding of the root cause, or, more likely, the multiple contributing root causes, that provides the basis for helping prevent an entire class of incidents. It also provides the basis for valid data analysis under programs of RCM.

Our stable of analytical techniques has been adapted to the systematic evaluation of incidents with the intent of uncovering the root causes.

We use boolean-based combinations of deductive (see, for example, our eFTA tool) and inductive (see the ePHA tool) methods to systematically analyze incidents. The incidents are ultimately resolved to a combination of root causes that are defined at a sufficiently basic level to allow practical remedial actions to be identified.

Having fully documented the evidence, investigation, and the identified root causes, we can also assist in the identification, documentation, planning, and implementation of corrective actions intended to prevent recurrence.
